To help you get the most out of your appointment with ABC Nutrition, we’ve put together a nutrition assessment form for you to complete before your first appointment. This will help us to gain a better understanding of your medical history, lifestyle and eating patterns. We are not here to judge - there are no right or wrong answers.
This form will be sent out to you via email, once you have booked in to see us. By filling out this form ahead of time, we can spend more time focusing on YOU and what really matters during your first consultation, rather than collecting information!

We offer a client-centered approach and will put your needs first! Your initial consultation is where we learn more about you and what makes you tick. We will review your completed online nutrition assessment with you and talk with your to understand more about what your needs and expectations are. Once we have an understanding of what’s important to you, we will work together to develop a set of personalised goals and strategies, based on your nutrition and health needs.
We believe ‘slow and steady wins the race’ - with small and gradual changes being more sustainable in the long-term. Your dietitian will work with you to break big goals down into smaller, more achievable actions. During this session, your dietitian will provide you with plenty of helpful information to help you reach your goals and also provide you with education about the importance that nutrition plays in various health conditions.

We typically don’t develop personalised meal plans as we find they can be too prescriptive and regimented. Instead, we believe it is important to be flexible and have no boundaries when it comes to what you eat. However, since we know you can sometimes get stuck for inspiration, we will provide you with plenty of nutritious meal and snack ideas that you can use at home.
If you prefer to have a personalised plan developed, we can do so for an additional charge, depending on your needs. Please let us know during your consultation, if you would like to receive this option and we can advise you of the associated fees. Alternatively, contact us and we can have a 15 minute call to discuss your needs.

Our follow-up appointments are where we focus on and refine your goals and actions even further. They are also an opportunity to provide more in-depth education and to provide additional help and support.
The pace you work at with your dietitian is entirely up to you, and because of this we offer a range of packages and programme tailored to suit your needs.
The Fine print
Service Agreement
When you book an appointment with ABC Nutrition, you agree to the terms set out in our Service Agreement. This will be emailed out to you at the time your initial consultation appointment is confirmed.
Payment
An invoice will be sent to you at the completion of your appointments with us. You can pay via online banking or Stripe (for credit card payments). Payments must be received by ABC Nutrition within 24 hours of your completed appointment.
Failure to make payment within the 24-hour period may result in your outstanding payment, being transferred to a Debt Collection Agency for retrieval. If you default on payment, you are liable to pay all costs associated with collecting payment.
Packages
All packages are to be paid in full at the time of purchase. Package payments are non-refundable and unused sessions are unable to be transferred or redeemed for cash. Any session canceled within 24 hours of the scheduled appointment time or no-shows, become void.
Cancellation & Rescheduling Policy
Helping you reach your health and wellness goals is really important to us. We appreciate that from time-to-time plans change and you may need to cancel or reschedule an appointment. Where possible, we kindly ask that you provide at least 24 hours notice, to allow other clients the opportunity to make an appointment. Otherwise we may ask that you pay a 50% cancellation or rescheduling fee.
Please note that for our packages, which are offered at a discounted rate, any sessions which are not attended or cancelled within the 24 hour notice period, will become void.
If you are running late, we may not always be able to complete the full consultation, but we always appreciate a phone call or text to let us know. This will help us accommodate any changes which may be required.
Insurance Rebates
Nutrition services provided by a NZ Registered Dietitian may be eligible for rebates from some private health insurance providers depending on your insurance fund or level of cover. Contact your health insurer to see if you are eligible.
